You need to provide your own plain white shirt (any brand but white with NO stripes, herringbone or french cuffs). Typically waistcoats, socks, and ghillie brogues are self-provided (purchased from St Kilda Retail, Booval) however we have limited sets available for short term (up to 12 months) loan.

I acknowledge that care and maintenance of all property on loan to me by COIPB is my responsibility and must be maintained to the same condition in which it is received

I acknowledge that any damage or loss must be reported to the COIPB executive within 7 days of the damage or loss, and that damage or loss of any item of equipment or uniform will result in a financial obligation on me to compensate COIPB within 30 days of the damage or loss occurring, in accordance with the listed retail value.

I acknowledge that negligent actions causing damage or loss may result in disciplinary action in accordance with the COIPB model rules and by-laws.

I agree to return all items of uniform and equipment within 7 days of any request for return by the COIPB executive. Where applicable (kilt, waistcoat, jacket, glengarry, etc.) this must be returned with proof that the item has been dry cleaned. Failure to dry clean items upon return will result in a charge being issued to cover the cost of COIPB having the item dry cleaned.

Please provide a copy of your Drivers License or other identity document. Note that your document is stored and protected under Australian Privacy Laws as Personally Identifiable Information (PII).
